If you would like to join our practice, but are unsure if we are participating providers with your insurance, please click here to check our list of accepted insurances. If you do not see your insurance on our list, please contact our office and check with the receptionist to see if we accept your insurance before making an appointment.
If you already know that we accept your insurance and you would like to make a new patient appointment, please call us at 912-353-7744 to schedule the appointment. You can arrive at your appointment about 15 minutes early to fill out paperwork or you can click here to download our new patient welcome packet and view our office policies. Please remember to bring your child’s most current insurance card with you to the visit.

We will ask you to verify your current insurance coverage at every visit. If your insurance coverage changes, please have your new insurance cards available at that time. Before each vaccination visit, find out if your insurance plan covers your child’s immunizations. Be sure to notify your employer/ insurance company about any new additions to your family! Please bring your most current insurance card when coming to our office to ensure that the insurance information we have on file (i.e. member id, group number, etc.) is correct at the time of service. Your claim processing could be delayed and you could be held financially responsible if the insurance information on file is invalid.
We collect your copay at check-in so that you may leave the office immediately following your appointment. This can help with your child’s comfort after vaccinations and saves you from having to wait around after your child’s visit.
Payment may be made with cash, check, Visa or Mastercard and is required at the time services are rendered. Your copay is determined by your insurance card/company, which you should bring with you to each visit.
